To begin the onboarding process we need to know if you’ve had recent blood work done or if you need us to order it for you. Having up to date lab reports is vital to customizing your treatment.

If you’ve had blood work done for testosterone levels within the last six months you can upload the lab report in the link below. If not, please indicate that you need blood work to be done. 

In addition, we also need you to upload your driver’s license for identification purposes.
